Its health inspections are equally bad, only 1 star (much below average). Inspections by State and/or Federal investigators have found 19 health deficiencies (as compared to an average U.S. facility having only 6.8 deficiencies). Staffing at Salem Center was also much lower than both the National and the West Virginia averages for registered nurse hours per patient per day, and for certified nurses’ aide hours per patient per day – again only 1 star (much below average).
